This sad fact was the <\/span><a href=\"https:\/\/apnews.com\/article\/kidney-transplant-race-black-inequity-bias-d4fabf2f3a47aab2fe8e18b2a5432135\"><span data-contrast=\"none\">case with the national kidney transplant list<\/span><\/a><span data-contrast=\"auto\">. For years, Black Americans in need of a new kidney were denied their spot on the transplant list because of an algorithm that calculated Black and non-Black patients differently. This occurred even though Blacks are three times more likely than Whites to have kidney problems. Disturbingly, similar formulations have been used to determine the treatment for heart disease, lung function, and even the delivery approach for pregnant women. How and why has \u201crace,\u201d a socially constructed and historically changing concept been used so egregiously? Let\u2019s use sociology to find out. <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The categories used to distinguish race in the U.S. have changed over time. In the 1970s, the U.S. Census used <\/span><a href=\"http:\/\/chrome-extension\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/\/www2.census.gov\/library\/publications\/decennial\/1970\/pc-s1-supplementary-reports\/pc-s1-11.pdf\"><span data-contrast=\"none\">three racial categories<\/span><\/a><span data-contrast=\"auto\"> to define people. The options were White, Black, and Other, which encompassed Indigenous, Asian, and Hispanic Americans. They don\u2019t tell you their culture, language, or religion, and they certainly don\u2019t tell you their kidney function or ability to give birth vaginally after a C-section. Yet, racial category formulas were used by kidney professionals for years to determine treatment. The result was that over <\/span><a href=\"https:\/\/apnews.com\/article\/kidney-transplant-race-black-inequity-bias-d4fabf2f3a47aab2fe8e18b2a5432135\"><span data-contrast=\"none\">14,000 Black people<\/span><\/a><span data-contrast=\"auto\"> were wrongly kept off the kidney transplant list because of a medical formula.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">It is important to realize that this discussion isn\u2019t about the role of <\/span><a href=\"https:\/\/www.bmc.org\/genetic-services\/ancestry-based\"><span data-contrast=\"none\">ancestry in genetic disease<\/span><\/a><span data-contrast=\"auto\">. For example, Blacks have a higher risk of <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/sickle-cell-disease\/\"><span data-contrast=\"none\">Sickle Cell\u202fDisease<\/span><\/a><span data-contrast=\"none\">, Ashkenazi Jews are at greater risk of <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/tay-sachs-disease\/#:~:text=Tay%2DSachs%20disease%20is%20a,people%20from%20other%20ethnic%20backgrounds.\"><span data-contrast=\"none\">Tay-Sachs disease<\/span><\/a><span data-contrast=\"none\">, and Europeans are more likely to have <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/cystic-fibrosis\/\"><span data-contrast=\"none\">Cystic Fibrosis<\/span><\/a><span data-contrast=\"none\">. Instead, this is about people being excluded from treatment because of their race even though they would qualify if they were of a different race.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Arguably, these policies could be best explained by the concepts of institutional racism and institutional discrimination. <\/span><b><span data-contrast=\"none\">Institutional racism <\/span><\/b><span data-contrast=\"none\">is defined as <\/span><i><span data-contrast=\"none\">societal patterns that produce negative treatment against groups of people based on their race.<\/span><\/i> <span data-contrast=\"none\">This is about deeply embedded beliefs, values, and norms that validate the unequal treatment of individuals and groups based on race. On the other hand,<\/span> <b><span data-contrast=\"none\">institutional discrimination<\/span><\/b><span data-contrast=\"none\"> is the<\/span><i><span data-contrast=\"none\"> use of social institutions to deny minority group members access to the benefits of society<\/span><\/i><span data-contrast=\"none\">. Broadly speaking, discrimination is about the actions taken against someone. In this instance, it means the <\/span><span data-contrast=\"auto\">government, corporations, or institutions are engaging in discriminatory practices against a minority group in society. Healthcare organizations using a formula to weed out medical recipients solely based on <\/span><span data-contrast=\"none\">their race surely fits the definition of institutional discrimination. Institutional racism and discrimination are often accompanied by <\/span><b><span data-contrast=\"none\">implicit bias<\/span><\/b><span data-contrast=\"none\">,<\/span> <i><span data-contrast=\"auto\">unconsciously held attitudes, beliefs, and stereotypes about others<\/span><\/i><span data-contrast=\"auto\">. An often-researched example is that of <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2793179#:~:text=Several%20studies%20have%20reported%20associations,discrimination%20with%20worse%20pain%20outcomes.&amp;text=Physician%20implicit%20bias%20has%20been,less%20pain%20than%20White%20patients.\"><span data-contrast=\"none\">pain treatment and management<\/span><\/a><span data-contrast=\"auto\">. Doctors have been found to offer their Black patients less pain management care than they offer White patients. This doesn\u2019t mean Black patients experience pain any differently than their White counterparts<\/span><span data-contrast=\"auto\">,<\/span><span data-contrast=\"auto\"> but that doctors let their bias about Black patients influence their decisions on offering pain treatment. Could this help explain why the <\/span><a href=\"https:\/\/www.pbs.org\/newshour\/health\/how-racial-inequity-is-playing-out-in-the-opioid-crisis\"><span data-contrast=\"none\">opioid epidemic in the U.S.<\/span><\/a><span data-contrast=\"auto\"> disproportionately impacted White Americans? Quite possibly. Some of the most egregious examples include<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<ul>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"2\" data-list-defn-props=\"{&quot;335552541&quot;:1,&quot;335559685&quot;:720,&quot;335559991&quot;:360,&quot;469769226&quot;:&quot;Symbol&quot;,&quot;469769242&quot;:[8226],&quot;469777803&quot;:&quot;left&quot;,&quot;469777804&quot;:&quot;\uf0b7&quot;,&quot;469777815&quot;:&quot;hybridMultilevel&quot;}\" data-aria-posinset=\"1\" data-aria-level=\"1\"><span data-contrast=\"auto\">In the 19th century, <\/span><a href=\"https:\/\/www.history.com\/news\/the-father-of-modern-gynecology-performed-shocking-experiments-on-slaves\"><span data-contrast=\"none\">gynecological experiments without anesthesia<\/span><\/a><span data-contrast=\"auto\"> were performed on enslaved Black women by Dr. James Marion Sims. Sims was the founder of The American Gynecological and Obstetrical Society (AGOS) and early president of the American Medical Association (AMA). <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<ul>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"2\" data-list-defn-props=\"{&quot;335552541&quot;:1,&quot;335559685&quot;:720,&quot;335559991&quot;:360,&quot;469769226&quot;:&quot;Symbol&quot;,&quot;469769242&quot;:[8226],&quot;469777803&quot;:&quot;left&quot;,&quot;469777804&quot;:&quot;\uf0b7&quot;,&quot;469777815&quot;:&quot;hybridMultilevel&quot;}\" data-aria-posinset=\"2\" data-aria-level=\"1\"><span data-contrast=\"auto\">Starting in 1932, the <\/span><a href=\"https:\/\/www.history.com\/news\/the-infamous-40-year-tuskegee-study\"><span data-contrast=\"none\">Tuskegee Experiment<\/span><\/a><span data-contrast=\"auto\"> involved Black men with the venereal disease syphilis being left untreated so the researchers could watch the progress of the disease. This experiment continued even though penicillin was discovered to cure the disease in 1943. The experiment didn\u2019t end until 1972. In the process, some went blind, others went insane, and many men died as a result of not being treated. Her tissue became known as HeLa cells and had unusual properties that resulted in them being used in countless research studies around the world. Lack\u2019s family sued and finally reached a <\/span><a href=\"https:\/\/www.npr.org\/2023\/08\/01\/1191283359\/henrietta-lacks-descendants-settlement-stolen-cells\"><span data-contrast=\"none\">confidential settlement in 2023<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<\/ul>\n<ul>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"2\" data-list-defn-props=\"{&quot;335552541&quot;:1,&quot;335559685&quot;:720,&quot;335559991&quot;:360,&quot;469769226&quot;:&quot;Symbol&quot;,&quot;469769242&quot;:[8226],&quot;469777803&quot;:&quot;left&quot;,&quot;469777804&quot;:&quot;\uf0b7&quot;,&quot;469777815&quot;:&quot;hybridMultilevel&quot;}\" data-aria-posinset=\"4\" data-aria-level=\"1\"><span data-contrast=\"auto\">In 2020, it was reported that <\/span><a href=\"https:\/\/www.cnn.com\/2020\/08\/18\/health\/black-babies-mortality-rate-doctors-study-wellness-scli-intl\/index.html\"><span data-contrast=\"none\">Black babies<\/span><\/a><span data-contrast=\"auto\"> have a higher mortality rate if treated by a White doctor than if cared for by a Black one. But when a life is at stake, even a few examples are too many. At the start of their career, new doctors take the Hippocratic Oath, pledging to do no harm. Nowhere in either the <\/span><a href=\"https:\/\/www.pbs.org\/wgbh\/nova\/doctors\/oath_classical.html\"><span data-contrast=\"none\">old<\/span><\/a><span data-contrast=\"auto\"> or <\/span><a href=\"https:\/\/www.pbs.org\/wgbh\/nova\/doctors\/oath_modern.html\"><span data-contrast=\"none\">new version<\/span><\/a><span data-contrast=\"auto\"> of the oath is the race of the patient mentioned<\/span><span data-contrast=\"auto\">, nor should<\/span> <span data-contrast=\"auto\">race <\/span><span data-contrast=\"auto\">ever<\/span><span data-contrast=\"auto\">. <\/span><span data-contrast=\"auto\">R<\/span><span data-contrast=\"auto\">ace<\/span> <span data-contrast=\"auto\">should n<\/span><span data-contrast=\"auto\">ever<\/span><span data-contrast=\"auto\"> be a deciding factor in treatment or how people are treated.
